What is a 245D license?
A 245D license is issued by the Minnesota Department of Human Services. It allows providers to offer Home and Community-Based Services to individuals with disabilities, including both basic and intensive support services.
What waiver programs do you accept?
We accept several Minnesota waiver programs including CADI, BI (Brain Injury), DD (Developmental Disabilities), and AC/EW (Alternative Care/Elderly Waiver). Contact us to discuss your specific waiver.
How does the Choice Referral Program work?
